Our Aquinas Healthcare nurses play a vital role in supporting both our clients and their families. At the start of each home care case, a nurse completes a comprehensive assessment, reviewing medical history, medications, physician instructions, and any current health concerns. This allows our team to develop a thoughtful care plan focused on maintaining the client’s health, safety, and quality of life. Our nursing team continues to monitor each client’s care through regular in-person visits at least every 60 days, along with additional communication with families as needed. Nurses also visit when a client’s condition changes or when additional support would benefit the client’s overall care.
Additional nursing services are available upon request, including medication management and medication set-ups. Aquinas Healthcare employs both Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) and will provide the level of nursing support that best fits each client’s needs.
